No kid wants to skip recess or school because they’re sick. Not sure if your kid is up-to-date on all their shots? No problem! Call their doctor and they will be happy to check for you, or you may call the immunization toll-free hotline at 1-800-232-4636 . Do you need to see a new doctor or want to know where your kid can get their shots? Go to https://nmhealth.org/resource/view/1247 for an interactive map. Just simply enter your ZIP code or address to find the nearest doctor. Also, while your kids are at the doctor’s office, ask the doctor to complete a well-child visit for them if one is due. If your kids play sports, did you know that a well-child visit is FREE and includes a sports physical? That’s right! Don’t delay—take your kid today! Communication is important to you and to us. That’s why we offer translation services if you need them. We want to make sure you have the information in the language or format you use so you can take charge of your own healthcare. We have bilingual staff and certified translator services if you want to use these services. These services are available to help you talk with providers, nurses, caregivers; learn about your health plan; and much more. Presbyterian’s translation and interpreter services include: ● ● Language translation ● ● Written materials translation ● ● Telephonic interpreters ● ● In-person interpreters ● ● American Sign Language interpreters ● ● Video remote interpreters On-the-go access to your health record MyChart is a tool that allows Presbyterian Medical Group (PMG) patients* to access parts of their electronic health record. From your computer, tablet, or smartphone, you can log in to your MyChart to: • Get test and lab results • View medication and allergy information • Request an appointment • View summaries of past visits • Send messages to your care team • Request renewals of prescriptions You may also use MyChart to view the electronic health records of your eligible dependents. To learn more and to sign up, visit www.phs.org and select “Access MyChart.” *MyChart is available only to patients with PMG providers. Patients who have been treated at one of our hospitals but do not have a PMG provider may still sign up for MyChart to view parts of their records. Translation and interpreter services It’s easy to request these services. All you need to do is ask for these services at any one of Presbyterian’s clinics or hospitals.
