www.phs.org/centennialcare 5 Baby Benefits is a no-cost Presbyterian Health Plan program that gives you rewards for going to your prenatal (before your baby is born) and postpartum (after your baby is born) appointments. Step 1: Earn a $150 gift card Go to your first prenatal visit. This visit happens within the first 14 weeks of pregnancy or within 42 days of enrolling in Presbyterian Centennial Care. Step 2: Earn a $75 gift card Go to regular prenatal visits (at least 10) during pregnancy. Step 3: Earn a $100 gift card Go to your postpartum visit within seven to 84 days after your baby is born. This visit is for your care. How to earn your reward: • Go to your visit(s). • Submit certificates for each step online at www.mypres.phs.org or by mail to: Presbyterian Health Plan, P.O. Plus, eye exams for children can find problems that can affect the way they learn and develop. Presbyterian Centennial Care covers certain vision care services needed to diagnose and treat eye diseases and to correct vision. Medicaid has specific guidelines for when eyeglasses and contact lenses are covered. Routine and medically necessary eye exams are covered by Presbyterian Centennial Care. ● Children under 21 years of age are limited to one routine eye exam in a 12-month period or when care is medically necessary (needed). ● Adults over 22 years of age are limited to one routine eye exam in a 36-month period. ● You do not need a referral for routine eye exams. ● You can also make an eye exam due to medical reasons, such as diabetes. Your Presbyterian Centennial Care vision services Make an appointment soon for routine dental exams (preventive care). Starting routine dental exams now will mean better lifelong dental health for you and your children. Some covered routine preventive care services include: ● Dental cleanings: One cleaning every six months for members under age 21 and for members 21 years and older who have developmental disabilities. One cleaning every 12 months for members 21 years and older. ● Molar sealants: Service is covered only for members under age 21. Only one treatment per tooth every five years. ● Dental varnish: Topical fluoride varnish treatments are covered twice per year for children between age six months through age 20. Treatments must be provided by either a Presbyterian Centennial Care in-network dentist or primary care provider. Your dental care provider can also tell you what preventive services will be covered by Presbyterian Centennial Care. Call your dentist today!
