10 Your healthcare wishes are based on what’s important to you, your beliefs, culture, and discussions with your loved ones. Advance care planning is the process of writing and talking about what kind of healthcare you would want if you were sick or injured and could not speak for yourself. It includes choosing someone to speak for you. What is an advance directive? Once you know your wishes and have chosen someone to speak for you, you can create your advance directive document. An advance directive is a legal document that stays in your healthcare record and has details about your healthcare wishes. It can be changed at any time. How do I start? We have an online advance directive form that is easy to fill out, edit, and submit to your Healthcare Team. When you complete your electronic advance directive, your document will stay in your medical record so that your wishes are easy to access for your doctor. When creating this document, there are three main steps: 1. Choose who you would want to talk for you if there ever is a time that you cannot talk for yourself. This person is called your healthcare decision-maker. Make sure you talk with them so they can agree to this role. 2. Fill out your online document. You can start and come back, as well as edit it later if something changes. 3. Submit the document online to your Healthcare Team.
